Location: Houston, TX (Gulf Freeway Area)
Pay: $26.00 per hour | Weekly Pay

Openwork is seeking a dedicated Housing Case Manager to support a community-based supportive housing program serving young adults ages 18–24. This role offers a meaningful opportunity to work directly with youth as they transition into stable housing and develop the skills necessary for independent living and long-term success.

As a Housing Case Manager, you will provide individualized support, guide participants through the housing process, and help them build the foundation for stability and self-sufficiency. The ideal candidate is organized, compassionate, and experienced in case management, with a strong commitment to supporting youth through life transitions.

Position Overview:
The Housing Case Manager works closely with program participants to identify housing opportunities, complete required documentation, and coordinate the steps needed to secure safe and stable housing. After placement, the Case Manager continues to support participants through ongoing case management, service coordination, and goal planning to ensure continued housing stability.

This position requires strong relationship-building skills, attention to detail, and the ability to collaborate effectively with internal teams and community partners.

Key Responsibilities:
  • Provide individualized case management services to youth participating in the supportive housing program.
  • Conduct housing readiness assessments and develop participant service plans.
  • Assist youth with housing applications and navigating housing voucher processes.
  • Support participants through housing briefings and documentation requirements.
  • Coordinate the lease-up and move-in process to ensure a smooth transition into housing.
  • Prepare and submit required documentation to local housing authorities, including initial applications, recertifications, move requests, income updates, and family composition changes.
  • Provide ongoing support and case management services once housing is secured.
  • Participate in wraparound service meetings to coordinate resources and support plans.
  • Maintain accurate and timely case notes and documentation in the case management database.
  • Collaborate with program staff to educate youth on housing readiness and available resources.
  • Support program events, workshops, and outreach activities when needed.

Education Requirements:
Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, Social Services, or a related field.

Required Qualifications:
  • Must have previous case management experience.
  • Strong organizational and documentation skills.
  • Proficiency with Microsoft Word, Excel, Outlook, and data entry systems.
  • Ability to build supportive, professional relationships with youth.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with multidisciplinary teams.

Preferred Experience:
  • Experience working with youth transitioning from foster care or supportive housing programs.
  • Familiarity with public housing programs, housing vouchers, or housing subsidies.
  • Knowledge of supportive housing programs and community-based services.

Schedule Requirements:
This role requires flexibility, including availability for occasional evening hours and weekends based on program needs.

Work Environment and Additional Requirements:
  • Regular in-person interaction with youth in a supportive housing environment.
  • Ability to exercise sound judgment and maintain professional boundaries.
  • Reliable transportation required.
  • Valid driver’s license and current liability insurance required.
